Hugh Chatham Memorial Hospital is responsible for conducting a Community Health Needs Assessment (CHNA) every 3 years from a diverse group of community partners and stakeholders to collect, analyze, and interpret data and identify the most pressing needs among residents of the region we serve. This report and its associated action plans are used to identify priority health issues and to plan interventions to build a healthier community.

Hugh Chatham Memorial Hospital es responsable de realizar una Evaluación de Necesidades de Salud Comunitaria (CHNA, por sus siglas en inglés) cada 3 años con un grupo diverso de socios y partes interesadas de la comunidad a fin de recopilar, analizar e interpretar datos, e identificar las necesidades más apremiantes entre los residentes de nuestra región a la que brindamos servicios. Este informe y sus planes de acción asociados se utilizan para identificar los problemas de salud prioritarios y planificar las intervenciones para construir una comunidad más saludable.
