How do I access?

Patients can self-register for the INPATIENT portal by going to MyCareCorner.net and creating an account. Once you have an account, you will need an invitation code to access your health record and those of your family members. Contact your healthcare provider to request a registration invitation for MyCareCorner.

Patients may request an invitation for the OUTPATIENT (medical group) patient portal by contacting the practice location directly.

What if I have questions regarding my medical record?

If you have questions about your medical records, or feel that an error has been made, please contact Hugh Chatham Health Information Department at 336-527-7376.

Who should I contact if I have trouble logging in?

If you are trying to access the Hugh Chatham Health Patient Portal, and you have already registered, please call the Patient Portal Helpline at 336-527-8413 Monday through Friday from 8:00am-4:30pm for assistance in resetting your login. If you are trying to access the Hugh Chatham Health Medical Group Patient Portal and you have already registered, please contact the practice directly for assistance in resetting your login.
