Our patient health portals make it possible to review and submit forms, view health information, download and transmit medical information to most providers, request appointments, and send messages to your healthcare providers.
Welcome to MyAtriumHealth – Your New Patient Portal
We want to make it easy for you to stay connected to your care. That’s why we’re moving your patient portal to MyAtriumHealth.
Your basic personal information – including your name, address and billing details – will be transferred automatically to MyAtriumHealth. Up to three years of past, current and future health records have been automatically transferred to MyAtriumHealth for continued access to your care information. If you don’t see the records that you are looking for, please contact 336-527-7376 (Option 4).
Accessing MyAtriumHealth
If you already have a MyAtriumHealth account, no new account or login is required.
If you need an account, it’s easy to create your MyAtriumHealth account online:
- You can sign in from here or download the free MyAtriumHealth app in the App Store or Google Play
- Under New User, select Sign Up Now
- Enter your personal information and answer a few questions to verify your identity
- Once you sign up, your account is automatically activated

Frequently asked questions
Yes, proxy relationships to children and legal dependents will automatically transfer to MyAtriumHealth. No action is required on your part.
If you discover that proxy access is missing, please follow the steps outlined in the FAQ question below.
If you are a child’s parent or legal guardian, you can view their medical information and manage their care in MyAtriumHealth through proxy access. Proxies of children between the ages of 0-12 have a full view of the child’s information, while proxies of children between the ages of 13-17 have teen access, which is a limited view of the child’s information by default. Like other health systems across the country, Atrium Health Wake Forest Baptist follows state and federal regulations that limit online access to patient records for individuals aged 13-17.
A full view of the child’s information is granted to proxies when a patient 13-17 years old is of diminished capacity – meaning the patient is not mentally capable of making informed medical decisions on their own. Diminished proxy access requires a doctor’s order. Due to certain medical needs, there are instances when a parent or guardian may request access to the child’s health record that will remain active after the child turns 18. As always, a parent or guardian may call the doctor’s office to discuss their children’s health concerns.
To access a child’s account, follow the below steps:
- Sign in to MyAtriumHealth.
- From Your Menu, select Request Family Access under Sharing.
- Then select Child Proxy.
Account access should be available within a few business days, and MyAtriumHealth will prompt you to confirm the child’s date of birth before access becomes active.
You will need your child’s:
- Social Security Number, or
- Atrium Health Wake Forest Baptist Medical Record Number (MRN). You can find the MRN on a receipt from a child’s visit, right above the home telephone number.
Yes, we take great care to make sure your health information is private and secure. MyAtriumHealth follows federal privacy laws (HIPAA) and uses encryption technology to protect your records.
Here are some tips to help keep your information safe:
- Choose a password that is easy for you to remember but hard for others to guess.
- Two-step (or multifactor) authentication is automatically turned on for all accounts to help keep your information secure.
Two-step verification is an added layer of security designed to protect the information in your patient portal account.
In addition to signing in with your username and password, two-step verification will ask you to enter a one-time-use security code. This helps us verify your identity. Once you successfully enter your code, you will be signed in.
If you choose to skip this step, frequent users will only need to complete two-step verification from time to time.
Two-step verification is turned on by default for all MyAtriumHealth users to help keep your health information private and secure.
MyAtriumHealth lets you choose how you’re notified for many different types of updates.
To manage your notifications:
- Sign in to the MyAtriumHealth app (iOS| Android) or website.
- Open the menu and under Account Settings, select Communication Preferences.
- Expand the list of notification options for the category you want to change.
- To turn off a notification, deselect the envelope icon or text balloon icon next to it. The icon will turn white when the notification is off.
- To turn on a notification, select the envelope icon or text balloon icon next to it. The icon will turn blue when the notification is on.
- You can also expand Advanced Settings within each category to choose preferences for more specific notifications.
- When you’re done making changes, make sure you select Save Changes.
