At Yadkin Valley Adult Medicine, we are specialists in applying scientific knowledge and clinical expertise to the diagnosis, treatment, and compassionate care of adults across the spectrum of simple to complex health issues. We are highly experienced in diagnosing puzzling medical problems, in the ongoing care of chronic illnesses, and in caring for patients with more than one disease. We also specialize in health promotion and disease prevention.
As internists, we specialize in the treatment of adult patients ages 16 and older. We provide care that includes disease prevention, as well as treatment of acute and chronic illness.
Our practice has been recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). This signifies that our medical practice meets national standards established to improve health through well-coordinated management of patient care.
The medical home is a model of care in which each patient has an ongoing relationship with a physician or provider who leads a team that takes collective responsibility for the patient’s care. In the medical home model, the primary care physician serves as a coordinator, directing other team members in their roles to ensure that every patient gets appropriate care at the right place, right time and right amount to support his or her optimal health.
